Requirements:

Must have been in business for at least One (1) year.

Must have a minimum of two (2) trucks preferably no older than five (5) years.

Must upload a digital copy of Provincial and/or State Operating Authority(ies).

Must upload a digital copy of insurance certificate, whereby minimum coverage is:

(i)               Motor Vehicle $1,000,000 ($2,000,000 if transporting Hazardous Materials)

(ii)             Public Liability $1,000,000

(iii)            Cargo $150,000

Note: On successful application, The FBI Group will be contacting your agent to request that FBI be named as additional insured on policy.

Must upload a digital copy of HAZMAT certificate (If Hazmat certified)

Enter applicable WSIB (Ontario) or Workers Compensation Insurance (statutory) in the space provided.

If US based, upload a digital copy of W9 form.

Must have direct contact ability for drivers via mobile phone, pager, or satellite system.

Must supply three (3) references from other Brokers that you have done business within the last six (6) months.
